Overview
Qatar is developing substantial capacity in terms of its scientific research and clinical practice. In recent years, a number of new hospitals have opened with world class facilities. Despite these developments, Qatar lags behind in mental health care. Unlike other areas, the psychiatry department has not been provided with new facilities to cope with the increased capacity it needs. That is not to say that progress has not been made. Community care launched six years ago with a referral-based centre offering a home-like environment, integrated services have launched in a few primary health centres, a forensic psychiatry community has been launched and a community team for people with intellectual disabilities is being formed. There have been five annual community care conferences. Notwithstanding the great progress made, it remains the case that mental health care is predominantly not dealt with at the primary care level in Qatar. At the same time, the Mental Health Law 16/2016 dealing with hospital admission and treatment has yet to be implemented. Underlying these challenges is the stigma that continues to be associated with mental health conditions in the country. This stigma stifles progress. At this time of transition, Qatar must undertake a multifaceted approach to the problems. First, it must continue to tackle the stigma and encourage greater awareness of mental health. Second, leaders must provide the necessary comprehensive investment needed for greater hospital admission capacity as well as primary care. The implementation of the Mental Health Law must be preceded by appropriate guidance and training given to clinicians in the health sector. That training ought to ensure that patients are predominantly dealt with in the primary care setting before hospital admission is considered. The appropriate facilities should be available for clinicians to undertake that role effectively. The guidance developed in conjunction with the law, ought to be updated frequently to ensure that it keeps apace with ethical knowledge and scientific advancements. Finally, important discussions are needed about ethical conflicts that arise and how to deal with them.
