Overview
This workshop will focus on 5 key aspects of medication safety highlighted by the gaps analysis survey. These are: polypharmacy and deprescribing, medication reconciliation, patient counselling, the "look-alike sound-alike' (LASA) medications, and medication safety incidents related to the COVID pandemic. During the workshop, the presenters from the three disciplines of pharmacy, medicine and nursing will lead case-based discussions and lectures covering the 5 key focus areas.

  1. 1- Recognize why and how medication safety incidences occur in the medication prescribing, dispensing and administration processes in primary care
  2. 2- Outline risk factors for adverse drug events and medication errors
  3. 3- Summarize key approaches for addressing prescribing, dispensing and administration errors in primary care
  4. 4- Outline strategies to reduce adverse drug events (ADEs) at interfaces of care admission, transfer and discharge
  5. 5- Recognize specific significant medication safety issues that arose in Qatar during the COVID-19 pandemic
